HD Lipo Podcast #8: Back Contouring With VASER lipo featuring Dr. Arian Mowlavi
Defining High Definition Liposuction of The Back Terminology
Dr. M: Back contouring refers to improving the overall contour or aesthetics of the backside, as you might think. The reason why we do back versus front is purely a surgical limitation in that we can either work on your front side as you might think because you’ve got to lay on your back or we can work on your backside. So, the back and the buttocks are on the side that we would potentially tackle in a high definition liposuction of the back.
FF: So when you say when you “lay down” you mean laying down on the on the table in the operating room and you work on our back as we lay on our front or work on our front as we lay on our back. Am I hearing you correctly?
Dr. M: Exactly, sometimes we do circumferential liposuction so that’s when you would be going all the way around. The limitations of that is that it takes about an hour to reposition, meaning if you’re going to start on the front side and then have to be moved to your backside and so forth, so patients may elect to do one side at a time.
Indications for High Definition Liposuction of the Back
FF: So when someone comes to you and you recommend back contouring, what are the issues they’re experiencing that lead you to believe that contouring will be the best solution?
Dr. M: So the question is, “What’s an aesthetically pleasing back?” and it has to do with the anatomy and configuration of the muscles and the bones on the buttocks. An aesthetically pleasing back is a little different between females and males – we study these things. For a gentleman, you want a really strong V line that is created by the latissimus dorsi muscle that lines the lateral border of your back and neck. The gentlemen were looking to have a very strong V line. The ladies’ V line may not be as important but there are two areas that are critical. Every woman wants a narrow waistline. They’re looking at a waste to buttocks differentiation. You can’t make the waist narrow enough. A lot of this has been popularized by actresses like the Kardashians. We want a narrow waist onset to a nice robust buttocks. The area that the ladies just don’t like is the roll in the mid lateral area.
FF: I can visualize the V-shape and the hourglass shape that these are most pleasing to others as they look at you.
Traditional Liposuction vs. High Definition Liposuction
FF: So I know you had previously worked with more traditional liposuction equipment and techniques. How has high definition liposuction made a difference in terms of the work that you do and the effects that you’re able to achieve?
Dr. M: With traditional liposuction we were able to contour the back, but with high definition liposuction we are able to get a more exaggerated look. For example, there is fat near the love handle lower back area for women. Men have unwanted fat there. It actually can add more to your waistline measurement than the tummy. Girls would come in and they got a beautiful tummy tuck. Their bellies are obviously tied down, but their waistline really didn’t shrink. And when you would look at them you would see this block of tissue wrapping around the back on that love handle and lower back. The patients want this removed. With traditional lipo, I would spend hours working on these love handles with traditional liposuction cannulas. And the problem is that the fat in this area is very fiberoptic. We’ve touched upon this before where we have superficial fat that is really tight and tethered by retaining ligaments while on the lower back area. These ligaments are not just in a superficial layer – they go all the way down to the muscle.
I would spend two hours with traditional liposuction tools and get this area down maybe 50-60 percent. With the advent of the high definition liposuction techniques and tools that we have at our disposal now, I am able to get that back bulk down by 90-95 percent. That’s how you get that exaggerated narrow waistline that can pop against a nice robust and appropriately shaped buttocks.
How Does High Definition Liposuction Work?
FF: I think this might be a good point to reiterate how the laser system works and how it allows you to achieve these more exaggerated results.
Dr. M: Sure. what we’re talking about is twofold. We are able to use VASER technology which is an ultrasound heat wave to literally melt the fat so that it’s liquefied and easier to remove. We are able to comprehensively remove fat not only from the deep but also the superficial layer. The second advantage is that because we’re liquefying the fat, we are able to remove fat at a more comprehensive level without creating contour irregularities. The problem with traditional liposuction is if you have eight hours, inevitably you would get the fat out, but you would be left with these contour waves and deformities of the skin because you couldn’t uniformly remove the fat and then allow the remaining fat to evenly distribute. So with traditional liposuction since the melters those semi-solid when you’re done liposuction and whatever you’ve left behind stays where it’s what. With VASER liposuction when you are done liposuctioning the remaining fat will evenly distribute. So you get these beautiful smooth contours.
FF: You’ve captured before and after photos that demonstrate how dramatic the changes are. Are there places where people can find these before and after?
Dr. M: I urge the listeners to go on google and just search for VASER Liposuction and High-Definition Liposuction. If they get lost they can go to highdefinitionliposuction.com where there is a lot of reputable physicians that are renowned in the world of VASER liposuction and they can get some pictures there. If you could see this before I would do a traditional liposuction and the patients would come in and they look at the results and they’d think it looks good. The difference is now I get a liposuction patient who comes in on their first week and when I take off their garment there’s a wow factor. They’re stunned. They can’t believe what they’re seeing.
FF: Well I’m certain that the before and after photos will tell the story and certainly the personal results each of your patients receives tells a big story for them and their loved ones. And when somebody is considering High-Definition liposuction, what sort of side effects might they want to be concerned about?
Side Effects of High Definition Liposuction
Dr. M: This technology involves removing unwanted fat. It’s superficial fat. There’s really no danger per se towards the VASER liposuction or high definition liposuction greater than traditional liposuction. In fact, it’s even safer. It’s actually a more gentle process because you’re melting the fat away from the structures such as the nerves and vessels. The overall surgical procedure is it’s just safer. Liposuction has its risks and they’re very well known, but High-Definition liposuction is actually a safer procedure than the old method.
FF: I can see that as it’s easier to remove the melted fat than the solid fat. The fat more easily pulls away from the fibrous material, nerve endings, and organs and it makes it much less injurious to the body.
Dr. M: Absolutely. And the back that we’re talking about today is just as important as the abdomen. There’s there’s fine detail work and you want to find a provider that really is an artist that can explain the differences between males and females and how they are going to achieve a narrower waist line so it looks more manly versus a narrower waist line so it looks more feminine. They’re very fine differences so all these things need to be detailed and planned for.
FF: That makes a lot of sense. You certainly want to come out with the with the correct effect based upon your gender.
Dr. M: For example, for a man you want to focus on defining their spinal column and defining their paraspinal muscles. The junction between the back and the buttocks is critical. So whereas in women we create a natural and gentle junction between the buttocks and the waistline, on men we actually want to create a shelf that makes it more manly. And the buttocks on a man needs to be square, whereas in a woman it’s more rounded. When talking about the ladies, we have the really narrow waist area and then a general transition area between that narrowness and the barracks. There’s an area right above the central buttocks area. We call this the lower back. For men, you don’t want to touch this area because he would look feminine. In the ladies you want to correct it and just take out everything. You want a beautiful diamond shape that’s positioned right above the barracks. And then you carry that around the box and into the transition zone and into the waist area.
Recovery with High Definition Liposuction
FF: Is there any more or less recovery time relative to other types of procedures that you do using definition liposuction on the back?
Dr. M: Contouring the back is just as safe and comfortable as any of the other areas. So I would say no you know that’s not a that’s not an issue.
In fact let me talk a little bit more about the love handle area because it is a problem for everyone. Before I had VASER liposuction and the cool helium plasma tool what I would be left with so much extra skin that I would have to commit myself to doing a lateral thigh and buttock tuck and actually have to cut the extra skin out. With the VASER, I’m taking out 20-30 percent more fat and there is even more extra fat. The cool helium plasma skin tightening tool allows me to not have to cut out skin and tighten it up. By using the High-Definition Liposuction techniques which include VASER technology, the power X cannulas, and cool helium plasma skin tightening we are able to achieve superior results without having a cut on people. The recovery is smoother and more comfortable.
FF: It never ceases to amaze me that you’re able to achieve dramatically better results quicker and with fewer side effects and less recovery time and discomfort.
Dr. M: It’s just such an amazing win.
The Surgical Process of High Definition Liposuction
FF: The process of doing High-Definition Liposuction for the surgeon is not easier. It takes time. Right?
Dr. M: You’ve really got to love what you’re doing because you could do a quick job and leave early or you could spend an extra hour or two and really sculpt out the detail of the muscle anatomy. It’s not quicker for me, but I know that I can achieve a result that was not possible before. The patient is able to achieve dramatically improved results, but overall the process is faster and easier for them from a discomfort and recovery time standpoint.
Cost of High Definition Liposuction
FF: Now how much does something like a back contouring procedure cost? I’m sure it depends upon how many different areas and how much work needs to be done.
Dr. M: So to do the upper, middle, and lower back and the flanks and some fat grafting to the buttocks and sometimes a little lateral thigh. It’s around $10000. And it’s about 30 percent more than traditional liposuction and it’s not because we want to charge more. It’s because it just takes more time. You have to take your time to do it right. That’s why it’s a little more pricey.
